7. NEW SERVICE CONNECTION :

7.1 (A) L.T. connection : (Lighting and commercial) :

Consumer has to apply in the prescribed form to J.E./A.E. in-charge of the Distribution Centre/town alongwith the following information.

a)

Name of the owner of house, House No., Ward/Area, Address.

b) 

Land/property/registration documents or payment receipt of property tax to Nagar Palika in his name.

c) 

In case of new constructed houses :

i) Letter of permission from Nagar Palika/Nagar Nigam alongwith approved map/drawing for construction of new house.

ii) Certificate of completion of house construction to be submitted before connection.

d) 

Purpose of light connection, domestic or commercial and total load (mention lights, fan points and plugs).

e)

Complete address and signature of consumer with date.

f)

In case of rented house requiring separate connection, no objection from house owner in addition to above.

7.1 (B) For L.T. Industrial/Commercial power connections :

 

(I)

Application form alongwith the following documents are to be submitted to J.E./A.E. (I/C) of Distn. Centre.

1.

Complete address of the consumer.

2.

Land documents, B.I., P-II, Khasra map (land ownership details alongwith map, site sketch).

3.

Sale deed of land.

4.

If the land is on rent or on lease then rent/lease dead shall be on requisite stamp paper and registered under Registration Act at least for 30 years.

5.

If it is partnership firm:

(a) Copy of registered partnership.

(b) Power of attorney on requisite stamp paper for authorizing the attorney to sign on application, agreement and other letters/under-takings.

6.

Permission/No objection from local bodies/Nagar Nigam/Nagar Palika/Gram Panchayat.

7.

Permission from Collector/Revenue authority/Town /country planning for establishing industry at the desired location of City/Town.

8.

No objection from pollution Control Board of the State.

9.

Copy of license from competent concerned Govt. authority.

10.

Financial status/Bank loan/finance documents.

7.1 (C) Agricultural connection :

 

1.

Application in prescribed form.

2.

Khasra map, Khasra record B-I and P-II from Patwari.

3.

Water availability certificate from Chhattisgrh Agro Ltd. or from ompetent agriculture officer.

4.

Distance certificate from nearest well to well/Tube well to Tube Well from agriculture officer.

5.

After feasibility survey, estimate is prepared by J.E. and submitted to A.E. After scrutiny estimate is submitted to  EE(O&M) for sanction.

6.

After sanction of estimate, deposit is to be made by the consumer within 30 days and agreement to be executed.

7.

After completion of formalities, work order is issued for execution of work.

8.

Consumer to install pump/tube well and submit the test report to concerned office for getting connection.
